on some of the reps it looks like your weight is being shifted from your heels to your toes, causing your knees to shift forward. just my two cents.
looks like too much forward lean and you look very unstable when you lower.

I watched your form and it use to do the exact same thing. Although its near perfect form, it is mandatory to perfect your form in the squat before you add weight. When I had similar form like you, I began to add weight. This started to irriatate my lower back (this would last anywhere from 2days to 1week). I asked a D1 football player to critque my squat form and I found out from his expertise that I was not keeping my chest up and that I was looking at the ceiling. When you squat bud, make sure you look straight forward...do NOT look at the ceiling. The 2nd correction (and after this your form will be perfect) is to keep your chest up. It seems like it is, but always make sure.
1) Look straight forward
2) Keep your chest up
3) With time, add moderate weight, like 10 lbs per week.
