Should I add protein and creatine on my off days? Or only on my work out days?

I like to add protein no matter what. Creatine I am not positive about, but I think you can moderate it a little.

You should take in enough protein to meet your daily requirements (generally 1 gram per pound of bodyweight), for most of us this means drinking 1-2 protein shakes everyday.
Creatine is most effective when dosed at 5 grams / day, everyday.

Like Tom said, 5g a day, every day. Read the creatine topics, it covers that.
Protein is just food. Asking if you should have protein is the same as saying "Should I eat chicken or steak on my off days". If your talking about powdered or liquid protein supplements then add them when you need to supplement you daily diet. Some days I have 100g of protein from powders other days none. It is just a supplement to hit my daily protein goal. That being said, you grow as your rest. So yes you want to keep your protein levels high on rest days so if you need supplements to do that then take them.

You heal on your off days too, so protein during off days is important.
