If you had this surgery and they removed the whole mammery gland would it be possible to ever have gynecomastia again from using AS?

I believe not, but often times they can't remove the whole gland without leaving a nice divot. So you might be left with some even after surgery.
Ultrasonic liposuction would also leave most of the glandular tissue.

Almost any gynecomastia surgery leaves some gland behind. Gland often presents as a concentration under the nipple and areola. However, it normally has fine fingers that extend into the surrounding fat. Look at this series of drawings to see how the gland can spread through the chest tissue (http://www.plasticsurgery4u.com/procedure_folder/male_breast/gynecomastia_anatomy.html). With my sculpture techniques, the remaining gland is usually insignificant. However any remaining gland can be stimulated to regrow. After gyno surgery that removes gland, there is much less gland to stimulate.
