Should you count the carbs in foods that are just sugar carbs? Such as milk?

Yes.

Count all carbs except fiber. I read somewhere that lactose will do the same as fructose, and replenish liver glycogen. Don't quote me on that. I don't remember where I heard it.

Either way it's an active carb, of that I am sure.

subtract fiber grams from total carbs. This is the amount of digestable carbs.
