Very poor choice of Drug for treatment.
drugs used less frequently to treat gynecomastia include danazol and testolactone.
Danazol, a synthetic derivative of testosterone, works by inhibiting pituitary secretion of LH and FSH, which decreases estrogen synthesis from the testicles. Dose used for gynecomastia is 200 mg twice daily. Complete resolution of breast enlargement has been reported in 23% of cases. Side effects include weight gain, acne, muscle cramps, fluid retention, nausea, and abnormal liver function tests, reduced testicular size as well as Testosterone production from the supression of LH and decrease in ejaculatory volume from FSH inhibition. Reduction in T levels can lead to erectile dysfunction althought not commonly reported
Testolactone, a peripheral aromatase-inhibitor, has been used with varying success rates (up to 40% decrease in size) in doses of 150 mg 3 times per day for 6 months. Nausea, vomiting, edema, and worsening of hypertension have been reported with its use.
Clomiphene, an antiestrogen, can be administered on a trial basis at a dose of 50-100 mg every day for up to 6 months. About 50% of patients achieve partial improvement in breast size, and about 20% of patients might note complete resolution. Figures vary with the type and duration of gynecomastia. Side effects include visual problems, skin rash, and nausea, and emotional changes.
Tamoxifen, an estrogen antagonist, is effective for recent onset and tender gynecomastia when used in doses of 10-20 mg twice daily. Up to 80% of patients note partial-to-complete resolution of gynecomastia within 3 months. Nausea and epigastric discomfort are the main side effects.
Heres a study worth looking at-
Comparison of Tamoxifen with Danazol in the Management of Idiopathic Gynecomastia ALBERT C.W. TING, M.B.B.S., F.R.C.S.,* LOUIS W.C. CHOW, M.B.B.S., F.R.C.S.,* Y.F. LEUNG,† M.B.B.S., M.R.C.PATH.†
Idiopathic gynecomastia, unilateral or bilateral, is a common physical finding in normal men. Successful treatment using tamoxifen (antiestrogen) and danazol (antiandrogen) has recently been reported. We compared the efficacy of tamoxifen and danazol in the treatment of idiopathic gynecomastia. We reviewed the clinical records of patients with idiopathic gynecomastia presenting to the Department of Surgery, University of Hong Kong, between August 1990 and September 1995. Medical treatment with either tamoxifen (20 mg/d) or danazol (400 mg/d) was offered and continued until a static response was achieved. The treatment response was compared. Sixty-eight patients with idiopathic gynecomastia were seen in the Breast Clinic. The median age was 39.5 years (range, 13--82), with a median duration of symptoms of 3 months (range, 1--90). The median size was 3 cm (range, 1--7). Twenty-three patients were treated with tamoxifen and 20 with danazol. Complete resolution of the gynecomastia was recorded in 18 patients (78.2%) treated with tamoxifen, whereas only 8 patients (40%) in the danazol group had complete resolution. Five patients, all from the tamoxifen group, developed recurrence of breast mass. In conclusion, hormonal manipulation is effective in the treatment of patients with idiopathic gynecomastia. Although the effect is more marked for tamoxifen compared with danazol, the relapse rate is higher for tamoxifen. Further prospective randomized studies would be useful in defining the role of these drugs in the management of patients with idiopathic gynecomastia.
If your gyno occured recently from elevated estrogen levels, Arimidex is another method of treatment.
