this has happened to me three times now and i just cant figure it out. when i train shirted bench and attempt a 1rm, the bar leans to the left and puts me off balance. i still have control of the bar but when i press it, i end up with the same issue again. i would think the shirt was put on incorrectly but im positive its more of my form. i had a partner watch the bar alignment and she tells me my left hand goes up first. im right handed if it matters.

anyone else have this issue or know what it may be?

Is it when you lock it out or right off the chest?

around 4" off the chest. ill get a video up in 2 weeks (my next attempt).

I had this problem raw for a while until someone pointed it out, tearing the bar apart helped out when I was still leaning good form, its hard to say without a video. Are you flaring too early with one arm?

Without a vid it's impossible to tell. Could be uneven sleeves, or that one sleeve is twisted. Could be just a strength imbalance. Could also be your technique.
