I know we've all seen some moron training who's so concerned about how much they're lifting and whose watching them that they almost end up killing themselves. What's the scariest thing you've seen happen in the gym?

I saw a guy once doing deadlifts with way more than he could ever handle. On his first set he did two shakey reps and couldn't rack the bar. He took the weight off to get the bar back on the rack and put double the weight on than the first set(if you could call it a set). He grabbed the bar and slid it off the rack and it crashed to the floor and it almost planted him head first like a lawn dart. He spent the next 20mins lying flat on a bench.

A buddy of mine saw a smith machine tip over with a guy doing squats with a lot of weight. The only thing that saved him were the 45's. How the hell did he tip a smith machine?

    I've seen a couple recently.

Power Cleans/presses using a Smith machine. (This same group of guys make me cringe when they do normal power cleans.)

A guy at 5'10", 150 pounds, doing 6 sets of high pulley cable curls (I think... they were sort of like half reps) with his sleeves pulled up so he can see his bulging 14 inch arms. I wanted to pick him up and carry him to the squat rack.

He made me laugh out loud (this happened last night).

LMFAO! Even though I only weigh like 155lbs, I squat and deadlift and don't do those gay exercises. I have seen people skinnier than the guy you mentioned doing the same things!

At my old guy there were these two guy that all they did was chest workouts. Monday I would go in, they would be doing bench press( 5-6 sets of them), then they would go to the incline bench do 5-6 set of that. Of course they are both those kind of guys that stick there chest out when they are walking around thinking they are big sh#t. Then every day for the whole week they work there chest, doing the same to things. It really makes me laugh.
