It's my understanding that you want to bring the bar down so it hovers over your clavical and then dip it back towards your chin? I'm sure one of the more experienced guys with correct me if I'm wrong. I know they are very difficult to do properly and a lot of guys that claim to be JM a lot aren't doing them correctly. However, Travis doing 405X3 on JM, now that's impressive

Where do you put your hands? It looks like most of them do about a thumb length from the smooth.
Depends on how big you are. I can't do them effectively any longer with my index on the smooth, but that is where I started
Here is the video of JM explaining it himself. As you can see if done the original way it's pretty tough to use huge weight. That being said I do them much like Travis and feel great benefit.
