When you bench press (working sets, not warmups), do you have someone help unrack the weight for you?
When the weight gets heavy, yeah I do. Especially if I have a training partner. No reason not to. It saves your shoulders big time as well as helps keep you in position
I try to save my lift off's for max attempts only. I feel it makes me "stronger" for my max attempts. If I can hit a 400lb rep max without a liftoff, then maybe the energy I would save with the lift off might give me 5 extra lbs.. you never know..

If I am training with someone then yes I get liftoffs for all my sets. This allows you to stay tight and keep a proper position. Saving for a max attempt is a bad idea since you won't be used to being set up like that and will most likely not be in the same position when you get the bar over your chest.

You will have gotten stronger when the weight that feels heavy is actually heavier than the weight that feels heavy now. Then the weight that feels heavy now will be a warmup for the weight that feels heavy then. But the weight will always feel heavy or you're not lifting enough weight. Clear? -Rippetoe
Yes, on anything 135 on up. A good liftoff lets you stay in position and save your energy for the lift.
