    LA Fitness Members - Need Help (Cancellation Form)

    Can someone please obtain a cancellation form from LA Fitness and scan/email it to me? I am unable to print it from the website and need to cancel the membership ASAP (there is not a local facility, I bought a week to week membership while traveling).

    I am going to try to contact their corporate office this afternoon, and if necessary will work through my bank.

    Was hoping that I could send a cancellation form as well as back up - but they do not allow you to print them online and I was instructed to print it from the website by the General Manager at the facility where I signed up.

    When I wanted my membership canceled at the powerhouse I was at, I just handled it directly with the billing company. Perhaps call that gym, find out who does their billing, and go from there.
