Definately cross train your grip: in addition to lifting specific work, such as deads and farmers, work with thick bars, grippers, etc. It will improve you grip faster than just lifting heavy with standard bars/DBs.

If I were to start from scratch as a young 13 year old again, I would do every press, squat, and perhaps deadlifts, for my entire career with chains. -- Dan John
I broke some bones in my hand when I was younger that never healed correctly so I have a problem with grip as well, I just try and train forearms to help with grip, but other than that when doing heavy shrugs I just use straps so I can hold the weight.
