How do I create a log?

I hear of alot of people having a log. I want to create a log to help me see results in numbers, and to help me feel a sence of accomplishment. Can someone tell me what their log consists of, or where I can find a site that i can get a sample from.

    etc

first number (225lbs) represents the weight, second one number of repetitions.
